Conditions · 5 min readNeck-related headaches: what an assessment may involve

Dr Adam WilliamsChiropractor · B.Sc M.Chiro · Updated 16 August 2026

Headaches are common, and they have many possible contributors. Some seem to start in the neck and base of the skull. Here's how I approach headaches in the studio — and when I'd send you somewhere else first.

Not every headache comes from the neck

Headaches that relate to the neck are sometimes called cervicogenic headaches. They're one type among many — alongside tension-type headaches, migraine and headaches with medical causes. Part of my job is working out whether the neck may be involved for you, and being honest when your history points elsewhere.

Starting with your headache history

The history is the most important part. I'll ask where the headache starts and spreads, which side it's on, how often it happens, how long it lasts, and what seems to bring it on or ease it. I'll also ask about neck stiffness, work and screen time, sleep, jaw clenching, past injuries and any medications. If you've kept a headache diary, bring it along.

What the physical assessment may include

  • How your upper neck and base of the skull move
  • Whether particular neck movements or pressure reproduce a familiar headache
  • Head, neck and shoulder posture, including at your workstation
  • Jaw movement and the muscles around the temples
  • Neurological checks, where appropriate
A careful history tells me more about a headache than any single test.

When a headache needs medical attention

Some headaches need urgent medical care rather than a chiropractic visit. Call 000 or go to the emergency department for a sudden, severe "thunderclap" headache. See your GP or seek urgent care for a headache with fever, a stiff neck or a rash; after a head injury; with weakness, numbness, vision changes or difficulty speaking; or a new pattern of headache, particularly over the age of 50. If anything in your history raises a concern, I'll refer you before going any further.

After the assessment

If your findings suggest the neck may be involved and chiropractic care is appropriate, I'll explain the options and how we'd review progress. That usually includes keeping track of headache frequency and intensity, so we're both looking at the same information at reassessment. If you also live with migraine, we can look at the neck component while you continue working with your GP or neurologist.

You can read more on our neck-related headaches page, or get in touch if you have a question before booking.
